What happens before the assessment?

Gathering as much background information as possible is an essential part of the process. Before the assessment can take place, you can expect an assessment pack containing information about me so that your child can feel at ease before the assessment. It will also provide details about the assessment location, instructions on what you need to do beforehand, and a list of items to bring with you on the assessment day.

Additionally, you will need to complete a parent questionnaire and your child's teacher (or whoever knows them best) will need to complete a school questionnaire. There is also a visual questionnaire to complete and I ask that your child has had an eye test within the last 24 months.

Please click the link provided below to download the questionnaires. Once downloaded, please forward to the class teacher or SENCO. These need to be returned before the assessment can take place.

Please note: The pack downloads as a .zip file containing both PDF and Word documents. We recommend downloading it on a desktop or laptop. If you have any issues, just get in touch and we can email the files to you. Only the Word (.docx) documents need to be completed and returned to claire@strive-dyslexiaservices.co.uk

What people say

Jenny M

Claire carried out a Dyslexia Assessment on my daughter. My daughter was super nervous but all went smoothly and she wasn’t stressed or dysregulated during or after the assessment.

A thorough in-depth report was received post assessment with lots of helpful information and advice on how to support our daughter with the challenges she faces going forward.

Thank you so much.

Claire completed a lot of research before the assessment which meant I was reassured she had understood my son’s situation well in advance. On the day of the assessment she made him feel comfortable and we then received a comprehensive follow up report and phone chat for any questions. I’d definitely recommend Strive and felt confident that Claire’s assessment gave us all the information we needed.
